Insurance

The registration fees do not include insurance of participants against accidents, sickness, cancellation, theft, property damage or loss. Participants are advised to arrange adequate personal insurance.

Final invoice

The final invoice will be sent to all participants by email after reception of the full payment.

Cancellation / Refund policy

Cancellation of registration

Until June 10, 2024 - 2000 CZK (eq. 80 EUR) administrative fee will be deduced from the refund.

From June 11, 2024 – transformation of registration from on-site to online participation will be possible and the difference between the registration fees minus 80 EUR administrative fee will be refunded.

Cancellation of Conference dinner and city tour

Until June 10, 2024 – 500 CZK (eq. 19 EUR) administrative fee will be deduced from the refund.

From June 11, 2024 – the conference dinner and city tour cannot be refunded.

General conditions

Any queries (changes, cancellations, etc.) must be notified in written by mail to bordetella@biomed.cas.cz according to the following conditions:

Transfer of registration to another person - a fee of 500 CZK (eq. 19 EUR) will be charged.

Registration for on-site participation will be closed as of April 30, 2024. Registrations received after this date will not be included in the list of participants in the Abstract book. Registration for online participation will be possible upon request until June 15^th, 2024.

I agree with the collection, preservation and processing of my personal data contained in this form by the administrator, the Institute of Microbiology of the CAS, v.v.i. and its employees, for the purpose stated below.

Purpose of processing personal data

In accordance with §5 of Czech Act No. 101/2000 Coll. on the protection of personal data, all information given in this form are collected and processed solely for the purpose of registration for the 14^th International Bordetella Symposium and for sending related messages to the data subject via electronic means pursuant to Czech Act No. 480/2004 Coll. Summarised data from this form may be used by the administrator for internal use and statistical purposes.

Statement by Organisers

The administrator declares that he will collect personal data to the extent necessary to accomplish the intended purpose and these may be processed only in accordance with the purpose for which they were collected. Individuals who process personal data are obliged to maintain the confidentiality of personal data, even after termination of employment or work.

Responsibility

The participant acknowledges that he/she has no right to lodge damage claims against the administrator should the holding of the 14^th14^th International Bordetella Symposium be hindered or prevented by unexpected political or economic events or generally by force majeure, or should the non-appearance of speakers or other reasons necessitate programme changes. With registration, the participant accepts this provison.

This consent is granted for all of the information contained in this form, and throughout the five years from the date of consent. At the same time, I am aware of the rights under § 12 and 21 of Czech Act No. 101/2000 Coll. on the protection of personal data.

I am familiar with all completed parts of this form, and I declare that all information are accurate and truthful and are provided voluntarily.
