Raising the Standard: Promising Practices in Geriatric Care

Conference Description

This virtual conference will feature promising practices in geriatric care, offering practical, evidence-informed education for healthcare professionals who support older adults across the continuum. Sessions will highlight strategies to strengthen person-centred, collaborative, and integrated approaches helping teams raise the standard for how they serve frail and medically complex older adults.  Speakers and topics were thoughtfully selected to support learners with varying degrees of experience in geriatrics, from front line clinicians to system leaders.

Objectives

At the end of this program, participants will be able to:

  1. Integrate promising practices that align with the Standard of Care to enhance outcomes for older adults living with or at risk of frailty.

  2. Integrate a person-centred approach that engages care partners and caregivers, and prioritizes what matters most to older adults in equitable, culturally appropriate care planning and delivery across the continuum.

Who should attend:

This conference is open to all health care professionals with an interest in geriatrics, including:

  • Physicians (Primary Care and Specialists)

  • Nurses (RN, RPN and Nurse Practitioners)

  • Allied health professionals and support staff (OT, PT, SW, SLP, PSW, OTA, PTA)

  • Health system leaders, administrators, managers, and directors

  • Medical learners (Students & Residents)

  • Community-based Providers, such as care coordinators, and EMS providers

Registration fees

  • All disciplines: $75.00

  • Students & Residents: $50.00


Cancellation Policy 

Cancellation Policy

Cancellations must be submitted in writing to the CEPD office at cepd@nosm.ca. Refunds will be issued minus the following administrative fees:

  • Up to October 18 : $25.00

  • October 19 to October 27: $40.00

  • October 28 to November 6: No Refund

This activity meets the certification criteria of the College of Family Physicians of Canada and has been certified by the Continuing Education and Professional Development Office at NOSM University for up to 5.5 Mainpro+® Certified Activity credits.
