Tackling Oral Health Inequalities through Link Working

Speaker: Eirian Kerry, Link Worker (Mouth Matters), GMMH NHS Foundation Trust Session overview: The session will discuss "The Mouth Matters in Mental Health Trial," a feasibility study aiming to support individuals with severe mental illness in accessing dental care. Conducted across three NHS sites in the North-West of England, the trial involved a nine-month link work intervention alongside treatment as usual for participants. Insights from link workers regarding trial experiences, successes, and challenges will also be reviewed.

Social Prescriber Role with High-Intensity Users in Primary Care

Speakers: Gina Goubran, Social Prescriber Link Worker, Central ,Thistlemoor and Thorpe road PCN/PCVS and Dr Neil Modha, GP Partner Thistlemoor Medical Center and Graham Young Pharmacy Session overview: Explore our award-winning project at Thistlemoor Medical Practice, highlighting the pivotal role of social prescribers with high-intensity users in primary care. Through a Population Health Management approach, our team engaged a cohort of 400 individuals with frequent A+E and GP visits, achieving a 70% drop in A+E utilization and a 30% decrease in GP visits. We'll share challenges, lessons, and the impact of partnering with local authorities and community services to address wider determinants of health, showcasing how social prescribing reduces pressure on the healthcare system.
