At the conclusion of this session, participants will be able to: • Recognize the common causes of postmenopausal endometrial thickening and their clinical significance. • Apply a stepwise approach to the initial evaluation, including when and how to use transvaginal ultrasound and endometrial biopsy. • Determine appropriate thresholds for referral to gynecology and understand key management pathways. Clinical Context • Prevalence of incidental endometrial thickening in postmenopausal patients •Increasing use of imaging and incidental findings •Why this matters: balancing vigilance with avoidance of over-investigation Normal Postmenopausal Endometrium • Expected endometrial thickness in asymptomatic women •Hormonal changes and endometrial physiology post-menopause •TVUS appearance of normal endometrium Differential Diagnosis of Endometrial Thickening • Benign causes •Malignant or pre-malignant causes •Atrophic endometrium with technical over-measurement The Role of Symptoms • Key question: Is the patient symptomatic (bleeding) or asymptomatic? • Significance of postmenopausal bleeding •Implications for work-up and urgency Stepwise Diagnostic Approach • Initial workup •Thresholds Role of Transvaginal Ultrasound (TVUS) • How to interpret stripe measurements •When TVUS is reassuring •Limitations (e.g., poor visualization, interobserver variability) •Consideration of repeat imaging or saline infusion sonohysterography (SIS) When and How to Biopsy • Indications for endometrial sampling •Techniques: Pipelle, office biopsy, D&C •Contraindications and when to refer directly for hysteroscopy •When imaging or clinical picture supersedes stripe measurement
