Direct Deposit Form - To be completed, signed and sent along with a voided cheque or direct deposit bank letter (found on your online banking account website).
Summer School Expense Claim Form - Spreadsheet to be completed (signature not needed) and submitted along with copies of detailed receipts, credit card statements (for any/all expenses paid by credit card), boarding passes, etc (refer to policies for detailed info)
